Who we are

Welcome to a new community partnership, working with East Cheshire NHS, CVS Cheshire East and Cheshire East Council, where we aim to link up people in our community with the time, energy and empathy to support lonely and isolated people where you live, to live a little better.

Our mission

  • We’ll be working with healthcare, social services and local charities to help knit together a comprehensive package of personal support which we’re offering first of all to people who are being discharged from Macclesfield hospital into our neighbourhoods from Congleton and to the north initially, and quickly moving to South of Congleton, around discharges from Crewe Leighton hospital.

    Most people who leave hospital after a stay of a few days, weeks or even longer, have good networks of families and friends and need nothing from us. However, many people don’t have this and would really benefit from someone turning on the heating before they leave home and making sure there’s some tea and milk ready for them, doing a little shopping or giving them a lift to a medical appointment. Just as important it someone who will stop and chat and get to know them as people, and help us to give them better support, whether through longer term befriending or by supporting their family carer, as they are often exhausted and need help too.

    We hope that this service will enable the hospitals to get people out of hospital a little quicker, and by making sure that someone in their community is helping, and checking in on them, will help their recovery and make readmission less likely.
